CDC recommends that all long-term care facilities and nursing homes take steps to improve antibiotic prescribing and reduce inappropriate use. Its Core Elements of Antibiotic Stewardship for Nursing Homes turns the Core Elements of Hospital Antibiotic Stewardship into practical ways to start or expand a stewardship program in long-term care (LTC).
The elements are the same for hospitals and nursing homes; how a facility carries them out depends on its staff and resources. The guide gives examples of how leaders, clinicians and staff can monitor and improve antibiotic use.
The seven Core Elements

| 1. Leadership commitment | shows support for, and commitment to, safe and appropriate antibiotic use |
| 2. Accountability | names physician, nursing and pharmacy leads responsible for promoting and overseeing stewardship |
| 3. Drug expertise | arranges access to consultant pharmacists or others with experience or training in antibiotic stewardship |
| 4. Action | puts in place at least one policy or practice to improve antibiotic use |
| 5. Tracking | monitors at least one process measure of antibiotic use and at least one outcome of it |
| 6. Reporting | gives prescribing clinicians, nursing staff and other relevant staff regular feedback on antibiotic use and resistance |
| 7. Education | gives clinicians, nursing staff, residents and families resources on antimicrobial resistance and on ways to use antibiotics better |
