If your low-density lipoprotein (LDL) cholesterol is high, your health care team may prescribe medicine along with lifestyle changes to bring it down.
Types of cholesterol-lowering medicine
| Type | How it works |
|---|---|
| Statins | Slow how much cholesterol the liver makes, and help the liver remove LDL cholesterol already in the blood. |
| Bile acid sequestrants | Remove bile acids, which the body makes by breaking down LDL cholesterol, and so help clear cholesterol from the blood. |
| Niacin (nicotinic acid) | A B vitamin that can improve all lipoprotein levels: it raises HDL ("good") cholesterol and lowers total cholesterol, LDL cholesterol and triglycerides. |
| Fibrates | Lower triglycerides in several ways, including cutting how much the liver makes; may also help remove LDL particles and raise liver proteins that can increase HDL. |
| Injectable medicines (PCSK9 inhibitors) | Lower the LDL cholesterol circulating in the blood. They are used for familial hypercholesterolemia, a genetic condition that causes very high LDL. |
Every medicine can have side effects, so talk regularly with your health care team, including your pharmacist. Once your levels improve, your team will keep checking them to make sure they stay in a healthy range.
Who may need medicine
Treatment depends on your cholesterol levels and your overall risk of heart disease and stroke. Your health care team may prescribe medicine if:
- you have already had a heart attack or stroke, or have peripheral arterial disease;
- your LDL cholesterol is 190 mg/dL or higher;
- you are 40–75, have diabetes, and your LDL is 70 mg/dL or higher;
- you are 40–75, are at high risk of heart disease or stroke, and your LDL is 70 mg/dL or higher.
Ask your health care team how you can lower your risk of heart disease. CDC explains high cholesterol, and Million Hearts has more on statins.
