The CDC funds state, tribal, local and territorial health departments to
run high-impact prevention programmes for **HIV, viral hepatitis, sexually
transmitted infections and tuberculosis** — implementing prevention,
addressing infectious diseases, and reducing disparities.

The stated priorities are **cost-effective, scalable interventions, policies
and research** to reduce incidence, and the money also reaches
**community-based organisations, national organisations, academic institutions
and education agencies**.

The part that is not money

Alongside financial assistance, **the CDC assigns federal employees with
on-the-ground experience to state, tribal, local and territorial health
departments.**

And **on request, the CDC can provide direct assistance — deploying public
health advisors or epidemiologists to help with disease investigations.**

That distinction matters. A grant lets a health department hire, if it can
find someone. An assigned federal employee with field experience arrives
already knowing how an outbreak investigation is run, and a deployable
epidemiologist arrives when one is already under way.

For a small health department facing a cluster it has never seen before, the
second kind of help is the one that changes the outcome — and it is the part
of the arrangement that a budget line does not show.

What the funding is for

Three things, in sequence: reduce infections, **link those who test
positive to care and treatment**, and prevent death. These programmes are
described as essential to the nation's public health infrastructure
which is to say they are not a project with an end date but the standing
capacity the country has for four diseases that do not go away.
