Medetomidine, a veterinary sedative, is **an increasingly common adulterant
of illegally manufactured opioids.** This report is about what happens when
people stop taking it.

What arrived at the hospital

During October 2024–March 2025, 23 adult patients who used illegally
manufactured opioids sought treatment within a Pittsburgh health system.

All exhibited severe autonomic hyperactivity — racing heart, soaring blood
pressure, agitation — and **most required dexmedetomidine infusion and
intensive care unit–level management.**

Dexmedetomidine is, essentially, the medical cousin of the drug causing the
withdrawal. Treating it took an ICU bed and a sedative infusion.

The testing problem

**Medetomidine metabolites were detected in all 10 patients for whom
retrospective analysis was performed** — but **only two had detectable parent
compound on comprehensive urine drug screening.**

A comprehensive screen, run at the time, missed it in eight of ten. The drug
had already been metabolised into something the test was not looking for.

What clinicians are asked to do

**Health care providers in regions where medetomidine has been detected in
the drug supply should be prepared to manage a severe withdrawal syndrome
among patients who use illegally manufactured opioids — even if drug testing
for medetomidine is negative.**

That is an unusual instruction, and it is the right one. A patient in severe
autonomic crisis with a negative screen will otherwise be worked up for
something else, or treated as opioid withdrawal — which this is not, and which
does not respond the same way.

The diagnosis here has to come from **what is circulating locally in the drug
supply** plus what the patient looks like, because the laboratory will not
supply it.
