Before you start
Outpatient dialysis facilities report to CDC's National Healthcare Safety Network (NHSN) under the Centers for Medicare & Medicaid Services (CMS) ESRD QIP rule. Full instructions are in the NHSN Facility Administrator Enrollment Guide, and common questions are answered in the FAQs about NHSN and the CMS rule. Once a facility is enrolled, more users can be added — at least two with administrator rights are recommended, to cover absences and turnover. New users should start with the new user checklist.
The steps

| 1. Train and prepare | Follow the enrollment checklist; complete the required training and the Outpatient Dialysis Center Practices Survey. Check that emails won't be caught by spam blockers. | 2 hours, 45 minutes |
| 2. Register | Agree to the Rules of Behavior and register the facility with its CMS Certification Number or CDC enrollment ID. Two emails follow: "Welcome to NHSN!" at once, and an invitation to register with SAMS within 24 hours. | 10 minutes |
| 3a. Register with SAMS | Open the link in the SAMS invitation, log in with the user ID (your email) and temporary password, complete registration and accept the SAMS rules. You have 30 days. | 16 minutes |
| 3b. Prove your identity | Follow the "Identity Verification Request" email, using either the online option (Experian) or secure document submission. | 35 minutes |
| 4. Activate your account | An approval email from SAMS ("SAMS Activity Authorization") links to the portal and the application, followed by a welcome email. Set up strong authentication: a soft token (the Entrust app on a phone, tablet or computer) or a hard token (an Entrust grid card mailed home). | 32 minutes |
| 5a. Complete NHSN enrollment | Once your token is set up, log in to SAMS, choose NHSN Enrollment, and complete the forms for your component. The facility administrator and primary contact receive "NHSN Facility Enrollment Submitted." | 40 minutes |
| 5b. Accept the agreement | The primary contact or facility administrator logs in to SAMS, selects the Dialysis Component, and accepts the "Agreement to Participate and Consent." | 5 minutes |
Things to watch
- A grid card should arrive by mail within 2 weeks of the SAMS approval email; if not, contact samshelp@cdc.gov.
- If you're approved for two or more applications, you'll get a separate email for each.
- The consent must be accepted within 60 days, or the facility is withdrawn.
After enrollment
Complete the NHSN dialysis training if you haven't, then go on to NHSN set-up, which is required before reporting begins.
