Leptospirosis is a zoonotic bacterial disease acquired from contact with
the body fluids of infected animals. It occurs worldwide and **can cause
severe illness in humans and animals.**
A human case was reported in Wyoming — **a historically low-incidence
state with a cold, semi-arid climate considered to lower the probability of
disease emergence.**
How it was solved
**Coordination by human- and animal-focused public health agencies
facilitated epidemiologic linkage of the case to a canine outbreak through
occupational exposure.**
A dog outbreak and a human case, connected because two agencies that do not
usually share a caseload compared notes.
Veterinary and human public health run on separate systems, separate
reporting and separate professions. A zoonosis crosses between them, and the
evidence that explains a human case frequently sits in the other system
entirely — as it did here.
Why the climate argument failed
Leptospira survives in warm, wet environments, so a cold semi-arid state is a
reasonable place to think it improbable. That reasoning is what the report
exists to correct:
**Leptospirosis should be considered in the differential diagnosis of
persons with occupational exposure to animals and clinically compatible
signs and symptoms — irrespective of geographic location.**
Irrespective of geographic location. The exposure is the risk factor, not
the map.
The symptoms to consider it against
Fever · chills · myalgia · nausea · vomiting · diarrhoea · headache · **calf
pain** · conjunctival suffusion.
Most of that list is any viral illness. **Calf pain and conjunctival
suffusion** — reddened eyes without discharge — are the two that should prompt
the question, in anyone who works with animals.
