During an Oregon measles outbreak running 11 June to 26 September 2024,
researchers went back and re-analysed archived regional wastewater
collected between 24 March and 22 September.
Wild-type measles virus was present in 20 of 94 samples (21.3%).
The ten weeks
**The first detection was in a sample collected on 3 April 2024 — ten weeks
before the first confirmed measles case.**
The outbreak included **a close-knit community that did not readily seek
health care**. That is the circumstance in which case-based surveillance
fails: the disease is circulating, people are ill, and the health system hears
nothing because nobody presents.
Wastewater does not depend on anyone seeking care. Everybody contributes to
it.
What it is good for
The report's conclusion is that **wastewater surveillance can provide an early
warning of community measles circulation, and can guide the public health
response during an outbreak — including recommendations for vaccination.**
Ten weeks is an enormous amount of warning for measles, which is among the
most contagious diseases there is. It is the difference between beginning a
vaccination campaign before transmission is established and beginning it
after.
The catch is that this analysis was retrospective: the samples existed,
and nobody tested them for measles until afterwards. The finding is not that
the warning was received — it is that the warning was there, in the archive,
unread.
Which is the argument for testing prospectively, in communities where
case-based surveillance is known to be weak.
