A veterinary anaesthetic is turning up in the drug supply

Medetomidine is a veterinary anaesthetic, and an emerging adulterant
detected in illicit drugs, drug paraphernalia and overdoses.

During August 2022–July 2023, it was **detected among five patients along
with xylazine, fentanyl and illicit opioids**. **No permanent sequelae were
reported.**

Xylazine — another veterinary sedative — went the same way a few years
earlier. Medetomidine appears to be following it.

The finding that is not about the drug

All five patients survived, and all received naloxone. Then:

Five people who had just survived an overdose. Three left hospital without the
medicine that reverses the next one, and four without a referral to treatment.

The emergency department did the acute job correctly in every case. The gap is
entirely in what happened at discharge — and that is the moment when someone
who has just nearly died is most reachable.

What the report asks for

**Medetomidine is an emerging adulterant detected in illicit drugs; further
investigation is important to better understand the clinical effects of
medetomidine and other novel adulterants.**

Because an adulterant that is not an opioid does not respond to naloxone. A
patient sedated by medetomidine or xylazine alongside fentanyl may have the
opioid reversed and remain deeply sedated — which looks like naloxone failing,
and is not.

**Programs to improve addiction treatment and naloxone distribution are
needed.**
