**Household contacts of people with pulmonary tuberculosis are at increased
risk of infection and disease**, and the honest starting point of this report
is that **effective interventions to improve case finding and increase the use
of preventive treatment are lacking.**

A pilot project at six health facilities in Uganda tried a package.

What it produced

Ninety-five per cent completion of a months-long preventive course is a
remarkable number for any health system.

What was in the package

• Home visits
• Chest radiography
• Adherence counselling
• Travel reimbursements

The fourth item is the one worth pausing on. Three of these are clinical; one
is money for a bus fare.

Preventive TB treatment fails at the point where a well person, with no
symptoms, has to keep travelling to a clinic for months to take medicine for a
disease they do not have. The cost of getting there is not a minor
inconvenience in that calculation — it is often the whole of it.

Including travel reimbursement in the intervention treats the barrier as what
it actually is, and the 95% completion rate is the result.

Why it matters beyond Uganda

Progression from untreated latent infection to disease is how most TB cases
happen. **Global scale-up of this household contact approach might help reach
global TB elimination goals** — and the 80 new cases found among contacts show
the case-finding half working alongside the prevention half.
