On 7 December 2022, Galveston County Health District notified Texas health
authorities of 10 consumer complaints of illness after eating oysters
nine raw, one smoked — at local restaurants between 27 November and 4
December. Symptoms began within 8 hours: diarrhoea, nausea, vomiting.
Initially, nobody sought medical care.

Oyster tags collected at three restaurant inspections pointed at **Oyster
Harvest Area TX 1 in Galveston Bay.**

The same day, from another state

The Florida Department of Health contacted Texas to report 37 cases of
gastrointestinal illness associated with raw oysters. **Their investigation
also identified TX 1 tags.**

Two states, one harvest area, the same week.

The fact that made it simple

**TX 1 had been the only area open for shellfish harvesting in Galveston Bay
since December 1, 2022.**

There was nothing to disentangle. On 8 December, Texas **closed TX 1 and
recalled every oyster harvested there between 17 November and 7 December.** On
9 December, Florida reported that **three clinical specimens had tested
positive for norovirus GII.**

The sex of 32% of patients was unknown, and **many reports were
anonymous**, which **made collecting further details or determining which
health department had jurisdiction difficult** — investigators used the
restaurant's address as a proxy when a patient's home address was not
available.

That is the quiet cost of a reporting system that depends on people
volunteering a complaint: it finds the outbreak and then cannot describe it.

Why the count is even this good

Reporting of sporadic norovirus cases is not required, but **outbreaks are
reportable** to both the state and CDC. So **norovirus outbreaks are primarily
detected when people report illness or a provider reports a suspected
outbreak** — roughly 2,500 a year in the United States. **A press release
led to additional consumer complaints**, which the report singles out as the
lesson: the announcement is part of the case finding, not only the aftermath.

Reopening

TX 1 stayed closed for 21 days while **water sampling and bacteriological
testing** were carried out from 20 to 28 December, following National
Shellfish Sanitation Program rules. **On 28 December the water met the
bacteriological standards and the area reopened.**

Meanwhile Texas compiled guidance for local health departments, restaurants
and consumers on norovirus transmission and disinfection, and **informed the
Interstate Shellfish Sanitation Conference, which notified other member
states** — the mechanism by which an oyster bed in Texas becomes eight states'
problem, and then eight states' information.
