You probably have a chart at your doctor's office, and perhaps at several; if you have been in hospital, there is one there too. These charts are your medical records, on paper or electronic. To keep track of all of it, it is a good idea to keep your own personal health record.
What to include
You could start with:
- your name, date of birth, blood type and emergency contacts;
- the date of your last physical;
- the dates and results of tests and screenings;
- major illnesses and surgeries, with dates;
- a list of your medicines and supplements, their doses and how long you have taken them;
- any allergies;
- any chronic diseases;
- any history of illness in your family.
