Brucellosis is a zoonotic disease caused by bacteria of the Brucella genus,
which people acquire from infected animals or their by-products. This is the
first US report since the **2010 Council of State and Territorial
Epidemiologists case definition** was adopted, and covers 2010–2024.

The data problem, first

Confirmed and probable cases reported through the national system1,796
Cases with a supplemental case report form878 (49%)
Of those, meeting the CSTE definition on the clinical and laboratory data supplied503 (57%) — 383 confirmed, 120 probable

**Nearly half the supplemental forms had to be excluded for want of adequate
clinical or laboratory data.** That is the report's own stated limitation, and
it comes before the findings for a reason: what follows describes 503 cases
out of 1,796 reported, chosen not at random but by which paperwork was
complete.

The two routes of exposure

Among the 503 cases that could be classified, **308 (61%) reported any
travel** — and of those, 245 (80%) reported only international travel in
the six months before symptoms began.

GroupMost frequent exposure
With international travelUnpasteurised dairy products or undercooked meat — 195 (73%)
Without international travelAnimal contact — 116 (62%), particularly skinning and slaughtering wild animals (47, 41%) and hunting wild animals (46, 40%)

Two different diseases, in effect, sharing one name in the statistics: a
foodborne illness acquired abroad, and an occupational or recreational
exposure acquired at home with a knife in your hand.

What follows from it

The report's recommendations track the two groups exactly:

  • Continued public health messaging to reduce exposure from
unpasteurised dairy products and undercooked meat, particularly those
produced outside the United States, and meat from wild animals
  • Safe field dressing practices among hunters — and among anyone else who
    skins or slaughters wild animals

And, underneath both: increasing awareness of brucellosis among **populations
at risk, health care providers and public health officials**, which is also
what would improve the data quality the report opens by apologising for.
