The Cholesterol Management Change Package (CMCP) is a Million Hearts guide (PDF) for outpatient clinics that want better cholesterol management. It was developed with the National Association of Community Health Centers (NACHC) and the American Medical Association (AMA).
The science of cutting cardiovascular risk keeps evolving, but there is strong evidence that a systematic approach to cholesterol management significantly improves care and outcomes. The CMCP helps practices build more efficient and effective systems for patients with hyperlipidemia.

They're organized in four focus areas: key foundations, equipping care teams, population health management, and individual patient supports.
How to use it
- Assemble a team — physicians, pharmacists, nurses, medical assistants and administrators — to identify the parts of cholesterol care that most need improving.
- Choose the interventions that fit those problems.
- Test each on a small scale first ("small tests of change") to check it's feasible and adjust before rolling it out more widely.
Don't try everything at once — and not every intervention will fit every setting.
The package draws on tools from many organizations, among them the American Academy of Family Physicians, American College of Cardiology, American Heart Association, the Department of Veterans Affairs, the FDA, the Institute for Healthcare Improvement and health systems such as Kaiser Permanente and Intermountain Healthcare.
